Staff-Patient Encounter Monitoring

Bridging the Visibility Gap: Automated Clinical Rounding, Equipment Integration & Interaction Insights

Traditional patient tracking requires months of setup. Cognosos changes the math. Operating in a “zero infrastructure” mode, our encounter-sensing tags use peer-to-peer communication to monitor interactions without requiring a location engine or heavy RTLS infrastructure. By leveraging staff badges, bed tags, and low-cost, disposable Bluetooth patient wristbands, our solution captures every interaction through simple proximity.

  • No location system required for encounter monitoring
  • No complex wiring or overhead
  • No continuous staff tracking, ensuring maximum privacy

Stop Installing. Start Improving. Traditional patient encounter tracking is slow, expensive, and hardware-heavy. Cognosos has stripped away the complexity to give you what matters most: Instant Visibility.

Traditional Patient Encounter tracking is slow, expensive, and hardware-heavy. Cognosos has stripped away the complexity to give you what matters most: Instant Visibility.

The Old Way The Cognosos Way
Months of infrastructure installation Instant out-of-the-box deployment using peer-to-peer communication
Expensive fixed RTLS sensors Zero location infrastructure requirements for encounter monitoring
Continuous staff tracking anywhere onsite Privacy-First: Encounter-only reporting; nurse-patient encounters are logged anywhere they occur
Manual workflow updates Automated EHR Integration and bed turnover notifications

The Cognosos Solution: Seamless Interaction Intelligence

Cognosos defines the new standard for encounter documentation. By utilizing low-cost Bluetooth beacons and automatic peer-to-peer sensing, we provide an indisputable, high-fidelity log of patients, staff, and equipment throughout the hospital, with or without dedicated infrastructure.

High-Fidelity Data

Our system automatically records the start, duration, and end times of all monitored interactions. When a nurse leaves the room and the proximity connection is broken, a report is automatically sent to the Electronic Health Record (EHR) to document the care encounter with zero manual data entry.

Comprehensive Equipment Integration

Bed tags detect mobile medical equipment (like telemetry units) and instantly associate them with the current patient. The system even tracks individual components using their own small Bluetooth beacons. For example, if a telemetry unit is missing its leads, an error is displayed until the leads are brought into close proximity.

Staff Privacy and Safety

To ensure privacy, Cognosos staff badges only report patient encounters rather than tracking staff full-time. However, by adding lightweight RTLS infrastructure, these badges transform into powerful safety tools. Pressing the badge’s duress button immediately notifies security and activates high-precision tracking for the duration of the incident.

Automated Bed Turnover

When a patient leaves the bed, the EHR is automatically notified that the room is ready for turnover. Simultaneously, the equipment in the room is flagged as “soiled” and scheduled for cleaning.

Key Benefits: Turning Interactions into ROI

Operational Excellence & Compliance

Automatically identify "gaps in care" and highlight patients who haven't been visited within a set window, ensuring 100% rounding compliance.

Reclaim Valuable Clinical Time

Give time back to your staff. Nurses no longer need to manually document "arrival" and "departure" times—the system handles the logging directly to the EHR so they can focus on the patient.

Empower Patient & Family Transparency

Improve HCAHPS scores by providing patients and families with a real-time "Timeline of Care" via bedside tablets or apps, proving their needs are being addressed.

The Invisible Gap in Your Hospital’s Operations

Today, most hospitals rely on manual logs or anecdotal evidence to track rounding compliance.

This “blind spot” creates a costly friction point:

  • Perception vs. Reality: Minutes can feel like hours to an ill patient, leading to lower satisfaction scores even when timely care is being delivered.
  • Compliance Fatigue: Asking nurses to manually “log a round” takes time away from the patient and invites human error.
  • The Data Void: Without automated tracking, leadership lacks the objective data needed to optimize workflow efficiency or defend against audit concerns.
